Abstract: Diabetes mellitus is a common disease with associated complications in multiple organs including the feet in what is known as “the diabetic foot”. The lymph drainage in the feet could be affected leading to edema, and predisposing to infection. The objective is to use Tc-99m nano-colloid lymphoscintigraphy of the lower extremities in patients with diabetes to study abnormalities of lymph drainage to provide an optimized method that could help in the management of these patients. A prospective study on 14 patients with diabetes and 10 non-diabetic patients was performed. Tc-99m nano-colloid lymphoscintigraphy of the lower extremities was done including whole body scans at 30, 60 and 120 minutes after intradermal injection of the radiotracer. Images were interpreted visually to define abnormalities in various aspects of the study including the number of lymph channels, time to visualization of the inguinal lymph nodes and others. A lymph drainage score was also calculated. The results showed various abnormalities in the lymphoscintigraphic studies in the diabetic patients including lymph dispersion (43%), collaterals (32%), and time-delayed inguinal node visualization (11%). The lymph drainage score was abnormal in 61% of diabetic patients. In conclusion, lymphoscintigraphy of the lower extremities in the way it was used in this research work could provide a simple means for gauging abnormalities of lymph drainage in diabetic patients which could enable early prevention of complications.
